We are proud to be in-network preferred providers with:

We also welcome patients with MetLife, Guardian, Cigna, Premera, Aetna, UnitedHealthcare, and most other dental insurance plans. Even if we are out of network with your insurance, you may still be able to use your out-of-network benefits at our office.
Don't have dental insurance? We also welcome patients without insurance and offer The Smile Space Dental Plan to help make quality dental care more affordable.
Our experienced team works with patients every day to help maximize both in-network and out-of-network dental benefits. Whenever possible, we'll help you understand your coverage and provide an estimate of your expected out-of-pocket costs before treatment.

We proudly accept The Smile Space Dental Plan by Clerri, LLC, a dental discount plan that makes it easy to get the quality, affordable dental care you and your family need and deserve.
The Smile Space Dental Plan keeps things simple: participating providers offer generous discounts directly to plan members. If you're ready to smile and save, it only takes a few minutes to enroll and unlock your dental discounts.

IMPORTANT CONSUMER DISCLOSURES
This plan is a discount plan and is NOT insurance coverage. The range of discounts for dental services provided under the plan will vary depending on the type of dental service received. Clerri, LLC, the discount plan organization, does not pay providers for dental services rendered to plan members. Plan members are individuals responsible for paying for all dental services but will receive a discount from those dental providers who have contracted with the discount plan organization.
NEW PATIENT FORMS
To help make your first visit as efficient as possible, please complete your secure online registration forms before your appointment.
Our online forms are HIPAA compliant, and your information is encrypted for your protection.
CANCELLATION POLICY
Your appointment time is reserved exclusively for you. We kindly ask that you provide at least 48 hours' notice if you need to cancel or reschedule your appointment. If you need to make a change to your appointment, please call us, text us, or contact us as soon as possible.
Appointments that are cancelled with less than 48 hours' notice, as well as missed appointments (no-shows), are subject to a $50 fee per hour of scheduled appointment time.
Patients who cancel late or miss an appointment may be required to pay a deposit before scheduling a future appointment. Repeated late cancellations or missed appointments may result in dismissal from the practice.
